Dr John Thanakumar, MS, MNAMS, FRCS, FRCS, Dip MIS(Fr), FIAGES Senior Consultant, Dept of Minimal Access, Bariatric and GI Surgery, Global Hospital, Chennai. Patient with Pheochromocytoma of the left adrenal gland diagnosed clinically with episodic hypertension, biochemically high catecholamines, positive MIBG scan and CT scan evidence of 6 cm large tumor underwent laparoscopic excision of the left adrenal gland. Patient was kept on the left lateral position. Primary trochar was subcostal with operating trochar to the right of the primary port. Ultra sound scalpel was used to dissect the vessels around the tumor. Left gonadal veins are identified along with the left renal vein. Junction of the left adrenal vein to the left renal vein is important. Initial alpha and beta blockers are required for the proper preparation for pheochromocytoma. Laparoscopic excision is useful in that the tumor handling is hardly present during surgery in addition to all the other advantages of minimal access surgery
